Get Ready for Fall! 2007-2008 Forms

Let's get ready for school! In order to prepare for the upcoming school year, we need some additional information from you. We have also posted additional information here to help you prepare for the upcoming school year.

The following forms are available online in a printer friendly Adobe PDF format. To access a form, click on a link and it will come up in a new window. You will be able to view, print and save the file from that window.

If you are unable to access the forms, please stop by the school to pick up hard copies.

Please return all completed forms to the school by July 27th.

Checklist

• Certificate of Immunization Form (one per student including returning students if you have had any new shots - for state reporting purposes - MUST be on this form per WA state guidelines)

• Vaccines Required For Elementary School Attendance Info Sheet (FYI)

• Vaccines Required For Preschool Attendance Info Sheet (FYI)

• Washington State Patrol Background Check Form (any adult that wants to work in the classroom)

• Parent Driver Information Form (one per any adult that wants to drive on a field trip)

• Authorization for Administration of Oral Medication Form

• Volunteer Signup (everyone!)

• Copy of Birth Certificate (new students only)

• Dennis Uniform Link (K-5th grade only - our school code # is XSCNAP)

• Open House and First Day of School Dates (everyone)

• Grade Level Supply Lists (K-5th grade only)
