Back-to-School Forms

On this page you will find the forms and information you will need to get ready for the 2010-2011 school year. These forms are available online in a printer friendly Adobe PDF format. Please print the forms, fill them out, and return them to the office.

If you are unable to access the forms, please stop by the school to pick up hard copies.

Please return all completed forms to the school by August 12th.

Checklist

• Consent and Release

• Health Questionnaire

• Washington State Patrol Background Check Form (any adult that wants to work in the classroom)

• Parent Driver Information Form (one per any adult that wants to drive on a field trip)

•  Certificate of Immunization Form (one per student including returning students if you have had any new shots - for state reporting purposes - MUST be on this form and signed per WA state guidelines)

• Vaccines Required For Elementary School Attendance Info Sheet (FYI)

• Vaccines Required For Preschool Attendance Info Sheet (FYI)

• Authorization for Administration of Oral Medication Form (signed by your physician)

• Volunteer Signup (everyone!)
